TERMS AND CONDITIONS

Information provided by New Dimensions Center for Wellness & Weight Control (hereafter referred to as NDCWWC) is for educational purposes only and is not a substitute for medical counseling or treatment. Please consult with your personal physician before beginning this, or any, diet.

Membership: At the end of the initial 3-month, 6-month or 12-month term, membership is automatically renewed each month. Members are responsible for all renewal fees, via PayPal, until notice of cancellation is received by NDCWWC.

Cancellation: Memberships can be cancelled at any time after the initial term. Cancellations must be received by NDCWWC at least 3 days prior to the member’s renewal date (the day the member signed up) in order to be cancelled for the upcoming month. For example, if the member signed up for a 3-month membership on January 15, the member’s renewal date is April 15, then the 15th of each month after that. NDCWWC must receive cancellation no later than the 11th of the month to stop that month's charge. Once the charge has been processed by PayPal, it is non-refundable.

To cancel, members may:

• Send email requesting cancellation, with your first and last name and membership number, to kymhaze@msn.com. Please do not send attachments because it is NDCWWC’s policy not to open e-mail attachments.
• Send a brief letter requesting cancellation, with your first and last name, membership number and e-mail address to: NDCWWC, 835 Bridger Cir, Craig, CO 81625

You agree to:

(1) create and maintain a PayPal account by signing up at www.paypal.com and providing credit card and/or banking information to PayPal (alternatively, all orders and payments can be handled by U.S. mail by sending orders with check or money order to NDCWWC, 835 Bridger Cir, Craig, CO 81625 - if you choose this method for ordering and payment, you must agree to processing time of up to 3 weeks for each order);
(2) maintain all equipment, software and Internet access necessary to use the NDCWWC web site (weight-control.com);
(3) protect the security of your confidential membership information (i.e., member web pages, membership number and passwords);
(4) be responsible for all charges, via PayPal, resulting from use of your NDCWWC account; and
(5) notify us of changes to your membership information, including e-mail address.

Waiver, Release And Limitation Of Liability: You agree that neither New Dimensions Center for Wellness & Weight Control (NDCWWC), nor its affiliates, subsidiaries, officers, directors, employees, agents, consultants or information providers or suppliers, shall have any liability to you under any theory of liability or indemnity in connection with your use of weight-control.org. You hereby release and forever waive any and all claims you may have against NDCWWC, its officers, directors, employees, agents, consultants, information providers or suppliers for losses or damages you sustain in connection with your use of weight-control.org.

Products sold on our website are non-returnable/non-refundable and should not be used as the only source of nutrition or nutritional information.
